At the National Readmission Prevention Collaborative Summit a few weeks ago, I spoke about the challenges health-care organizations face today in facilitating effective care coordination and how addressing those challenges can impact the patient experience.

Namely, when discharge planning and care coordination are successful, hospitals and health systems are better equipped to navigate patients through the care continuum, reducing readmissions and ultimately improving patient outcomes. Health Care IT Streamlines Transitions and Supports Discharge Planners

With staffing shortages and an influx of patients, there often aren’t enough care coordinators or discharge planners to efficiently facilitate care transitions, leaving some patients vulnerable to preventable readmissions and other negative outcomes.

And now, the discharge planning team must expand its reach. The Affordable Care Act requires them to focus their attention on what happens to patients after they leave the hospital.

Technology Puts the Package Together to Improve Care Coordination
