Many health systems are creating a network of post-acute providers that agree to meet quality standards, share data, provide certain services and work with hospitals to reduce avoidable hospitalizations.

Although skilled-nursing facilities and other post-acute care providers do not face the same financial penalties under the Affordable Care Act as hospitals do, there are still plenty of incentives for keeping patients out of the hospital.
